　粟粒結核の併存した脊椎カリエスの報告は比較的少ない．今回，われわれはその1例を経験したので報告する．粟粒結核を併存しないPott麻痺を伴う胸椎レベルの脊椎カリエスの外科的治療は，前方固定術が選択されることが比較的多い．しかし，今回の症例には粟粒結核の併存による肺機能低下があった．手術侵襲によるさらなる肺機能低下を危惧し，初回に除圧術単独を行い，追加手術として後方固定術を実施し，症状改善を得た．

　Cases of tuberculosis of the thoracic spine with miliary tuberculosis are rare.　Generally, tuberculosis of the thoracic spine is treated by anterior spinal fusion.　However, the patient's condition may become worse if anterior spinal fusion with differential lung ventilation is performed under bad pulmonary condition.　We successfully treated a patient with such a case.
