小児前腕両骨骨折術後に環指，小指深指屈筋腱癒着を来した1例

Adhesion of the Ring and Small Flexor Digitorum Profundus after Open Reduction of Fractures of Both Forearm Bone in a Child

　今回われわれは小児前腕両骨骨折に対する観血的整復術後に環指，小指深指屈筋腱癒着を経験したので報告する．症例は12歳の女性で，走っている時に転倒し左前腕両骨骨折を受傷した．同日，徒手整復とギプス固定を行った．その後，骨折部の再転位を認めたため観血的に整復し1.8mm Kirschner鋼線で固定を行った．初回手術後20日目に環指，小指の伸展障害を認めた．手術にて環指，小指深指屈筋腱の骨折部骨膜との癒着を剝離し伸展障害は改善した．原因として手術操作による骨折部骨膜との癒着が考えられた．

　We report a case of adhesion of ring and small flexor digitorum profundus after open reduction of a fracture of both forearm bones. The patient was a 12-year-old girl who fractured both left forearm bones when she fell while running. We initially performed closed reduction and immobilized the forearm with a cast, but nine days later the fractures were found to be displaced, and open reduction was performed . Twenty days after the first operation, the patient complained of being unable to extend her ring finger or small finger. At surgery we released adhesion of the ring and small flexor digitorum profundus from the periosteum. The adhesion of the ring and small flexor digitorum profundus was a postoperative complication of the first surgical procedure.
