 We investigated the relation between sagittal lumbar pelvic alignment of 53 healthy adult volunteers in the standing position and the sitting position by measuring the following parameters on lateral lumbar-spine radiographs:lumbar lordotic angle (LLA), sacral slope (SS), pelvic tilting angle (PA), pelvic morphologic angle (PRS1), and distance between the L1 plumb line and the center of the hip (L1-hip offset). There were no marked differences between the PRS1 angle in the standing and sitting position, because PRS1 is a constant value in each individual and determines pelvic orientation. The average changes in LLA, SS, and PA from standing to sitting position were -17.2°, -18.0°, and 18.6°, respectively (all p<0.01). In the sitting position, lumbar lordosis decreased, and pelvic rotation became posterior. The correlation coefficients for the relation between L1-hip offset and LLA and PA were r=-0.29 (p<0.05) and r=-0.54 (p<0.01), respectively. There was a close correlation between L1-hip offset and LLA.
