抄録:関節リウマチによる上位頚椎病変に対して環軸椎間スクリューや椎弓根スクリューを用いた内固定手術を行うにあたり,コンピュータナビゲーションシステムを使用した.造影CTを使用することにより椎骨動脈の走行もモニターした.19症例の術後画像をCTにて評価した.スクリュー誤挿入による神経血管合併症はなく,高い精度でスクリュー挿入を行うことができた.矯正損失もなく,良好な固定性が得られた.本システムは,術前のプラン作成や手術の安全性向上に有用である.
In this study the authors demonstrated that frameless stereotaxy can be applied safely to upper-cervical lesions caused by rheumatoid arthritis (RA). Instrumentation surgery performed with an image-guidance system was used to treat 19 RA patients with atlanto-axial instability between February 2000 and the end of January 2002. Neural and vascular injury was evaluated, and postoperative computerized tomography was used to determine the accuracy of screw placement. There were no neurovascular complications, and screw placement was highly accurate. No correction loss or instrumentation failure occurred. Image-guidance systems are useful tools in preoperative planning and in transarticular and pedicular screw placement in the cervical spine of RA patients.
