　低リン血症性ビタミンD抵抗性くる病（X-linked hypophosphatemic rickets：XLH）に対する脚延長は，日本ではいまだ報告されていない．今回われわれは，XLH未治療の成人女性に対し，脚延長術を施行したので報告する．症例は26歳の女性で主訴は低身長であった．延長法はOrthofix脚延長器を用いた，仮骨延長法（De Bastiani法）を適用した．延長距離は右大腿骨45mm，右けい骨24mm，左大腿骨40mm，左けい骨13mmであった．しかし，右けい骨で仮骨骨折，左大腿骨では25°の内反変形，左けい骨ではピン刺入部の感染症を合併した．長期的な経過観察と多くの症例を検討することにより，XLHに対する脚延長の治療法の改善が必要であると考えられた．

　This is, to our knowledge, the first report of leg lengthening in Japan involving a woman with adult X-linked hypophosphatemic rickets (XLH). The patient was 26 years old and short. For leg lengthening, we chose De Bastini's method using an Orthofix fixator. Elongation was 45mm in the right femur, 24mm in the right tibia, 40mm in the left femur, and 13mm in the left tibia. Complications included callus fracture of the right tibia, 25° varus deformity in the right femur, and pin infection in the left tibia. Further experience and longer follow-up are thus needed to improve leg lengthening in XLH.
