 A 35-year-old man with neurofibromatosis type 1 and severe dystrophic kyphoscoliosis of the cervical spine who was successfully treated using only a posterior fixation with a rod and lateral mass screw system is presented. Radiography showed a 93-degree kyphotic deformity, and MR images showed severe spinal cord compression at the C5 to C7 level. During the first operation, posterior fixation from C0 to T2 was performed;however, the patient had a restricted aperture. During the second operation, we removed the rod and screws from C0 to C2. After 3 years and 8months, he has no complaints regarding his daily life. Our results show that C0-C2 angle was important in the cranio-cervical posterior fixation and that this patient could be successfully treated using only posterior fixation.
