肛門括約筋傷害—Parks式括約筋修復術の方法と成績

Anal sphincter injury. Management and results of Parks sphincter repair

　肛門括約筋の断裂は外傷，肛門手術，産道外傷によりひきおこされ，さまざまな程度の排便障害をもたらす．これらの障害は患者を社会より遠ざけ，医師を訪れるまでの期間が長期にわたるものが少なくない．

　肛門括約筋修復術として多くの術式が報告されてきたが，それらを分類すると，間接法と直接法に分けられる．間接法は，他の部位の横紋筋または筋膜移植による補強や外括約筋縫縮術である．直接法は，断裂筋断端を遊離し，端々縫合することであるが，縫合部の膨開，筋断端の牽縮等により，悲観的結果に終つている．そこで，縫合法を重畳法に変えることにより良い成績をあげることができるようになつた．われわれは1961年から1982年に治療された97例の外科的治療につき述べる．

The late Sir Alan Parks had completed this review of his anal sphincter muscle repairs and the results were presented to the British Society of Gastroenterology in September 1982(Brown-ing GGP and Parks AG:Gut 1982:23:A901-2).All of the operations were perfomed by him.
