　左大脳半球深部白質の多発性梗塞により混合型超皮質性失語を呈した80歳女性を報告した。自発語に乏しく，呼称，言語理解，読み書きは著しく障害されていたのに対し，復唱は比較的保たれ，反響言語的であった。頭部MRIにて左角回を含む左頭頂葉皮質下の梗塞巣を含む両側脳室周囲の多発性梗塞巣が確認されたが，両言語野皮質には梗塞巣を認めなかった。SPECTでは両大脳半球には全般的な，右小脳にはcrossedcerebellar diaschisisの所見と考えられる局所的な血流低下を認めた。また，脳波では左半球のα波出現率の低下を認めた。本例では，左大脳半球の広汎な機能低下が示唆され，これが本病態の発症に関与したものと推察された。

We report a case of mixed transcortical aphasia (MTA) due to multiple cerebral infarction in the dominant hemisphere in an 80-year-old right-hand-ed woman without hemiplegia. Her spontaneous speech was markedly reduced and auditory compre-hension, reading and writing were severely disturb-ed. Although the repetition of sentences (at most 3 words) was relatively preserved, her speech was echolalic.
