　右への頭部回転運動によりめまいと眼振が速やかに誘発され，頭部回転時の脳血管撮影で左椎骨動脈が環椎軸椎関節部で閉塞していたbow hunter's strokeの66歳，女性例を報告した。本例に対して環椎の左横突孔のunroofingによる椎骨動脈の除圧術を行い，術後，頭部回転に伴うめまいと眼振は完全に消失し，脳血管撮影でも左椎骨動脈の閉塞は解除された。本例のように頭位変換によって回転と反対側の椎骨動脈が環椎軸椎関節部で閉塞し，椎骨脳底動脈領域の虚血症状を来すbow hunter's strokeは稀な疾患であり，その病態，治療法などについて文献的考察を加えた。

A case of vertebrobasilar insufficiency with ver-tigo and horizontal nystagmus induced by neck rotation is reported. This patient's angiography demonstrated obstruction of left vertebral artery at the C1~2 level on right rotation of the head. Surgi-cal decompression of the vertebral artery by partial transversectomy of the atlas vertebra in this case relieved the symptoms, and postoperative angiogra-phy demonstrated the disappearance of the verte-bral artery occlusion at the neck rotation. The pathogenesis and treatment of vertebral artery occlusion at C1~2 level are discussed.
