　症例は59歳，右利き男性。左内頸動脈閉塞により左下前頭回，島回，中心後回に病巣を持ち失語症を呈した。単語レベルの聴覚言語や文字言語，また日常会話の理解は良好であった。発語は構音障害は認めなかったが，自発話で喚語困難を伴い発話量は少なく，呼称・復唱・音読では字性錯語が多く認められたことより非流暢性伝導失語と診断した。本例の単語の呼称・音読・復唱は目標語が長くなるほど成績が低下し，また単語の後方に位置する音韻ほど誤りが多かった。本研究では課題を復唱に限定し，2拍から5拍の単語の復唱結果を構成音韻別に検討した。その結果，本例では復唱の際の音韻の選択・系列化・構音過程への変換が目標語の「長さ」の負荷により影響を受けたと考えた。

We report a case of non-fluent conduction aphasia. The patient was a 59-year-old right-handed male. He suffered from aphasia after a left internal carotid ar-tery occlusion. MRI study revealed subcortical lesions in the left inferior frontal gyrus and cortical lesion in the anterior part of the left insular gyrus and the left postcentral gyrus. The patient showed good compre-hension of words and daily conversation but had a common difficulty in the following tasks ; naming of pictures, repetition of words, reading of kanji and kana letters.
