間欠的光刺激によって誘発された要素性の発作性幻視の起源について—臨床発作症状，発作時脳波，発作時SPECT，脳血流定量による検討

　症例は43歳，女性。入院当初から視野の異和感，左手の痺れを訴えていたが，幻聴，妄想などの活発な精神症状のため，心気的な症状として考えられていた。しかし，光刺激により左視野に幻視像が誘発され，脳波上も右後頭部に持続性に徐波群発が出現したため，右後頭葉てんかんと診断された。発作後半に発作性恐怖が出現したが，これは発作放電が側頭葉内側部に波及した結果と考えられた。発作時SPECT像では右後頭葉皮質の一部と左小脳皮質に高灌流域が認められ，さらに1回動脈血採血法による脳血流定量では右後頭葉で対側と比して79％以上の脳血流量値の上昇が認められた。てんかん性精神病の既往がある場合でも，幻視，特に要素性の内容に対しては，後頭葉起源の発作性幻視の可能性も考慮する必要があると考えられた。

A 43-year-old, right-handed woman experienced right hand paresthesias and a visual field abnormality. We attributed her symptoms to psychiatric abnormali-ties, due to the presence of delusions and auditory hallucinations. Upon photostimulation, she experi-enced left visual field hallucinations and demonstrated slow waves on the right parieto-occipital regions. The clinical and electro-encephalographic findings sug-gested that these episodes were epileptic seizures originating from the right occipital region. Ictal fear appeared at the end of the seizure, reflecting the spread of seizure activity to the mesial temporal re-gion.
