てんかんの直接導出脳波—第1報：発作波の起源

ELECTROCORTICOGRAM AND SUB CORTICOGRAM OF EPILEPTICS

I．はじめに

　てんかんの症例について，露出した大脳皮質および皮質下から，脳波を直接導出することは，いわゆるてんかん波を局在づけようとする最終的な方法である。そして，それはてんかんの外科的治療の有力な助けになるばかりでなく，同時にてんかん波の正確な情報をうる手段である（この報告は，約10年間の中京病院外科における309例のてんかん症例の直接導出脳波の記録とその経験に基づいている）。

Sporadic single-spikes and so called spike and wave complexes seen in the primary epileptogenic focus, are usually observed on the electrocorticograms. Some of them have high amplitudes and rapid durations are observed on the scalp electrograms as a common type of epileptic discharges through the medium of volume conductor.

Two types of particular fast discharges seen in the primary epileptogenic focus are observed on the elec-trocorticograms. Paroxysmal high voltage fast discharges are shown in the atrophic lesions which are compara-tively large and incompletely destroyed such as the cases of porencephaly.

Spike-afterdischarges evoked by single-spikes are shown in the primary epileptogenic focus within or without organic lesion.

It would be summarized that these two types of fast discharges mediate between single-spikes and clini-cal convulsive seizure discharges.
