　多系統萎縮症（MSA）長期観察例14例中5例で8回の悪性症候群発症が観察された。4例で錐体外路症状が強く，うち2例はそれぞれ2回と3回発症していた。発症は5回が夏期であった。抗うつ薬によるのは1回，抗パーキンソン病薬服薬減少によるのは2例2回，感染などの合併症に続発が2例で3回，高体温に続発は2例で2回であった。自律神経障害の強い1例ではdroxidopa投与後に発熱した。転帰は死亡1例，腎不全1例で，自律神経症候の顕在化1例，元の状態への回復は4例で5回であった。抗パーキンソン病薬の服薬中断ないし減量以外に，感染などの身体的ストレスや，高体温が引き金になりやすく，悪性症候群はMSAの臨床上で注意を要すると思われた。

Five of fourteen patients with multiple system atrophy (MSA) experienced a total of eight episodes of malignant syndrome, three episodes in 1, two episodes in 1 patient, and a single episode in each of the other patients. Four patients had extrapyr-amidal symptoms and required antiparkinson ther-apy, including dopaminergic agonists. Five episodes occurred in the summer season. Two were caused by decreased or irregular doses of antiparkinson drugs, one by administration of an antidepressant drug, three by complications, and two by elevation of body temperature of environmental origin.
