　髄膜腫はくも膜細胞に由来し，その多くは硬膜または脈絡叢に付着して存在する。しかしそのいずれにも関係のない髄膜腫としてdeep sylvian meningiomaが知られている。本疾患は文献上11例の報告があるがMRI所見について記載されたものは見当たらない。我々は側頭葉てんかんで発症した本疾患の1例を経験し，MRI所見につき興味ある知見を得たので報告する。症例は62歳の女性で約1年前から意識消失発作が出現した。CTとMRIで左側頭葉内側部に拇指頭大の石灰化腫瘤を認めた。さらに造影MRIで腫瘤に接する中大脳動脈が帯状に増強され，髄膜腫に比較的特徴的とされるdural tail signに類似した所見が得られた。左前頭側頭開頭にて腫瘍を全摘出した。腫瘍はシルビウス裂内で側頭葉内側面に埋没する形で存在し，中大脳動脈と強く癒着していた。硬膜や脳室とは関係がなかった。病理検査にてpsammomatous meningiomaと診断された。

A rare case of deep sylvian meningioma is presented. A 62-year-old woman was admitted to our hospital because of one year history of temporal lobe epilepsy. She had no neurological deficit except for EEG abnormality. CT scans showed a small calcified mass in the left temporal lobe adja-cent to the sylvian fissure with no enhancement by contrast medium. The mass was low-intense in both T₁-and T₂-weighted MR images. The T₁-weighted image after the infusion of gadolinium revealed enhancement of the middle cerebral artery adjacentto the mass, similar to dural tail sign.
