MRIにて視床・被殻・小脳に両側対称性の病変がみられた急性脳症の1例

Acute Encephalopathy with Symmetrical Lesions of the Thalamus, the Putamen and the Cerebellum on Magnetic Resonance Imaging

　症例は，19歳男性。発熱・頭痛にひき続き，意識障害が出現し，さらにてんかん重積状態となった。頸部後屈と除皮質肢位を認め，髄液では，軽度のリンパ球優位の細胞増多と蛋白上昇を認めたが，糖は正常だった。血清・髄液のウイルス抗体価に有意な上昇は認められなかったが，ウイルス性脳炎を疑診し，アシクロビルを投与し，てんかん重積に対し，抗てんかん薬の内服に加え，ペントバルビタールの持続点滴も併用した。その後，てんかん発作は著明に軽減したが，失外套状態で固定した。入院3ヵ月後のMRIで，視床背側部・被殻・小脳の一部に，T₁およびT₂強調画像で，両側対称性に高信号域を示す病変がみられ，点状出血と考えられた。小児に，類似の臨床・画像所見を示す脳症が知られ，本例の発症機序に類似の病態を推定した。従来，MRIで本例と同様な所見を示した成人の急性脳症ないし脳炎の報告はなく貴重な症例と思われた。

A 19-year-old boy was admitted to our hospital because of impaired consciousness after experienc-ing fever and headaches for 7 days. The patient was in a deep coma or generalized convulsions, with retrocollis and decorticate posture. He never regained consciousness, remaining in an apallic state. The initial CSF pressure was 290 mm H₂O, CSF cell count 18/3 mm³, protein 75 mg/100ml, and sugar 74mg / 100ml. There was no significant elevation in the viral titer in the serum or CSF. Plain CT on admission showed marked brain edema.
