An autopsy case of a 36 year old male with ab-normal intracranial vascular networks containing an aneurysm was presented. He experienced the first attack of subarachnoid hemorrhage (SAH) at the age of nineteen and died a week after the third attack of SAH.

Macroscopically, marked hypervascularization was observed on the convex surface in the territory of bilateral middle cerebral arteries (MCAs). Arterial caliber of the Willis' circle was irregular. Using the operating microscope, the following findings were observed in detail. Bilateral MCAs disappeared at the point distal 1 cm from the internal carotid bifurcation. Abnormal vascular networks demon-strated by angiography were proved to consist of anterior choroidal arteries, perforating arteries and branches of anterior cerebral arteries. They did not so increased in number but showed marked dilatation and tortuousity. Anastomoses betweenthem were also seen. The aneurysm of about 0.5 cm in diameter, which was situated in the abnormal vascular networks and not revealed by postmortem cerebral angiogram with barium sulfate, was found to be fed by one of perforators arising from the left internal carotid artery. The cause of death was thought to be the rupture of the aneurysm into the ventricles.

Histologically, stenosis or occlusion of the above mentioned main arteries at the base of the brainwas due to fibrous thickening of intima without inflammatory or atheromatous changes. Marked concentric wrinkle of internal elastic membrane and atrophy of tunica media were seen at the stenotic segment of the arteries. These findings were most conspicuous in bilateral MCAs and ACAs.

Based on these findings, abnormal vascular net-works at the base of the brain seemed to form a collateral circulation.
