Consideration of the pathological, biochemical and pharmacological evidences investigated in the cases of Huntington's chorea and Parkinsonism led us to propose a hypothesis that there could be a dopamine-acetylcholine imbalance in the basal ganglia of both conditions.

Either to treat surgically or medically the invol-untary movements, we have to attempt to correct the imbalance between dopamine and acetylcholine.

Since dopamine seemed to be dominant in the cases of Huntington's chorea, the authors have tried to block the nigro-striatal pathway for decline of striatal dopamine in a case of 44 years old woman. The effect of stereotactic nigrotomy on the involuntary movement of Huntington has been exceeding than that of stereotactic VL thalamotomy performed previously.

Clinical courses of the case and theoretical back-ground of the nigrotomy were described.
