A case of 35 y-o female with direct arteriovenous communication between right extracranial arteries and intracranial dural sinuses is presented. The occipital artery and a meningeal branch of the vertebral artery supply an anomalous vascular network around the tentorium which drains into the sigmoid and transverse sinuses. Symptoms con-sisting of intracranial bruit, headache and vomiting took place following severe head injury in this patient.

Although such arteriovenous communication re-presents a dural arteriovenous malformation localiz-ing close to the tentorium, similar cases with traumatic origin or those arose following spinal tap have been reported.

From a study of 47 cases from the literature the authors emphasize that it is necessary to examine angiographically not only carotid arterial system but vertebral arteries when arteriovenous fistula in this area is suspected, because multiple source of blood supply is expected in these anomalous circulations.
