There seems to be some divergence of opinion on the indication of shunt operation to the so called Normal Pressure Hydrocehalus. Further study on the absorption mechanism of CSF is required to give some lights on this point.

Clinical study is carried out to compare ventricu-lar size of immediately after and 24 hours after PEG, together with estimation of CSF pressure before and 24 hours after "Pressure Injection PEG". (Fourth International Congress of Neuro-logical Surgery)

The result will be summarized as follows. In nomal case and in certain type of ventricular di-latation, size of lateral ventricles shows no change after 24 hours, and rather rapid lowering of CSF pressure is seen within 24 hours. In some cases of hydrocephalus, enlargement of ventricles is evi-denced. Rapid and remarkable lowering of CSF is not experienced.

Causative factor which brings about these two types concerning CSF pressure and size of ventri-cles is attributable to the disturbance of absorption mechanism of CSF.

The fact above mentioned leads author to con-clusion that shunt operation is indicated in the case which shows further enlargement of the ventricle and without remarkable fluctuation of the CSF pressure after PEG.
