We have reported the results of surgical treatment of 72 cases of hypertensive intracerebral hemorrhage.

1. These patients were divided into three groups by year of admission as follow : Group A was 16cases treated by aspiration of hematoma. Group B was 16 cases treated by evacuation of hematoma through trephanation. Group C was 40 cases treated by evacuation of hematoma including 17 cases performed external and/or internal decompression.

2. Mortality rate of each group was 44% (group A), 31% (group B) and 18% (group C) respectively.

3. This lowering of mortality rate was caused by such a factors as follow : 1) The diagnosis of acurate site and size of hematoma was achiebable by detailed analysis of cerebral angiograms. 2) Early surgical intervention was atempted before brain stem herniation developed by careful obser-vation of clinical symptoms. 3) Improvement of operative method was intended by total evacuation of hematoma, complete hemostasis and external and or internal decompression.

4. The priniciple of operative criteria was described based on our series.

5. Our operative results clearly show that the constructive and early surgery is effective method forserious cases.
