Cross Circulationを応用した選択的中大脳動脈造影法

SELECTIVE ANGIOGRAPHY OF THE MIDDLE CEREBRAL ARTERY

まえがき

　近年カテーテル法の普及にともなって選択的血管造影が広くおこなわれ，診断能力の向上に貢献していることはよく知られているとおりであるが脳血管については内頸動脈，外頸動脈の選択的造影は日常おこなわれるようになったが，脳固有の動脈の選択的造影は牧野らの研究がおこなわれているがまだわれわれの日常診療に用いられるまでには至っていない。脳動静脈奇型のfeeding　arteryの関係を明確にするため，あるいは脳血行不全等の血流動態をより明確に把握するためにも，前大脳動脈あるいは中大脳動脈の選択的造影の必要性を感じさせる場合は少なくない。

　ふるくから，1側頸動脈写に際して反対側頸動脈を圧迫することにより，前交通動脈を介してのcross circul—ationを利用して両側脳血管造影をおこないうることはよく知られている。われわれはこれを逆に応用して選択的中大脳動脈造影法を試み，日常診療にも利用しうることがわかったのでその方法を紹介する。

The value of selective angiographies will be more and more appreciated in the neurosurgi-cal practice as well as other fields of medicine.

A new technique of the selective arterio-graphy of the middle cerebral artery was peesented. The internal carotid artery is cannulated with a elastic needle and the ipsilateral carotid artery is compressed at the level proximal to the punctured point while the contrast medium is injected, so that the ipsilateral anterior cerebral artery is filled by the con tralateral carotid artery via cross-circulation.

We are applying this technique to the diagnosis of arterio-venous malformations and allied diseases. This method has been proved useful in identifying feeding arteries of arterio-venous malformation which presumably re-ceives vascular supply both from the anterior and from the middle cerebral arteries. How-ever, successful films are sometimes unobtainable in the cases whose anterior communicating artery is not patent or has a exterme individual variation.

This is a simple and safe procedure with wider applicabilities.
