The value of selective angiographies will be more and more appreciated in the neurosurgi-cal practice as well as other fields of medicine.

A new technique of the selective arterio-graphy of the middle cerebral artery was peesented. The internal carotid artery is cannulated with a elastic needle and the ipsilateral carotid artery is compressed at the level proximal to the punctured point while the contrast medium is injected, so that the ipsilateral anterior cerebral artery is filled by the con tralateral carotid artery via cross-circulation.

We are applying this technique to the diagnosis of arterio-venous malformations and allied diseases. This method has been proved useful in identifying feeding arteries of arterio-venous malformation which presumably re-ceives vascular supply both from the anterior and from the middle cerebral arteries. How-ever, successful films are sometimes unobtainable in the cases whose anterior communicating artery is not patent or has a exterme individual variation.

This is a simple and safe procedure with wider applicabilities.
