Four cases of traumatic extradural hematoma, in which extravasation from middle meningeal artery was demonstrated angiographically, were presented. And some discussions were added, about ;

1. Posttraumatic interval prior to angiography

2. Manifestation of middle meningeal artery and differentiation from other extra- or intracranial vessels

3. Characteristic figure of extravasated contrast media

4. Reference to operative findings, site of bleeding vessels, its relation to skull fracture, and extent of extradural hematoma.

5. Differential diagnosis of meningeal extravasa-tion

6. Significance of extravasation on angiogram in management of acute head-injured patients.

The careful observation of extravasated contrast media corresponding to disclosed torn meningeal arteries in operation, indicate that manifested extra-vasation refers tovarious circumstances, mostly to repair process of the injured vessel wall, quality of extradural coagulation and, in some cases, to hemo-dynamic mechanisms at each angiographic procedure.

Extravasation from middle meningeal arteries is mostly shown on the initiate bled vessels, which induce strip of dura from inner table of skull. In this reason, the obtained meningeal extravasation in angiogram provides the significant role on manage-ment of preoperative patients.
