A number of cases of internal carotid thrombosis, insufficiency or occlusion have been reported in lite-rature, among which the authors have recently noticed that there is a special type of abnormality of cerebral vessels. Characteristics of its angiographic findings are:occlusion or stenosis of bilateral internal carotid artery at the end of its siphon (C1) and bilateral hemangiomatous vascular network at the base of brain. The authors experienced 3 cases of this lesion and carefully reviewed and collected similar cases which were reported under various diagnoses in Japan. Thus total 24 cases were statistically studied.

This type of cerebral vascular abnormality is mos-tly seen in children and young adults, and clinical symptoms are motor and sensory paresis which is often transient, mental retardation, headache, uncon-sciousness associated with or without subarachnoid hemorrhage, convulsive disorder and involuntary movements. These symptoms are mostly mild and often transient.

Angiographic findings are; in addition to the above-stated, stenosis or occlusion of bilateral anterior as well as middle cerebral arteries. And, even if visu-alized, they are usually of poor filling, and they do not originate directly from the internal carotid, but from the vascular network. Size and grade of vi-sualization of the posterior cerebral artery is variable in carotid angiography, but often its dilatation and abundant branches are observed. Ophthalmic arte-ries are normally seen.

The authors have presumed that this type of cere-bral vascular abnormality may be of congenital ori-gin.
