血管腫様奇形破綻による硬膜下血腫の1例

RUPTURE OF AN INTRACRANIAL ANGIOMATOUS MALFORMATION WITHIN THE SUBDURAL SPACE

I．はじめに

　脳血管腫様奇形（angiomatous malformation）は，特発性脳内血腫の原因として近来注目されている。諸家の報告によると特発性脳内血腫の40〜60％にこの奇形をみるという。

　しかしangiomatous malformationは時に硬膜下血腫を来すことがある。こうした硬膜下血極が頭部外傷に引続いておこり，外傷性硬膜下血腫と診断，開頭により血腫と出血源と思われる小さなangiomatous malformationを確認，摘出した興味ある症例を経験したので報告し，多少の検討を加える。

A case of a subdural hematoma caused by the rupture of an intracranial angiomatous malformation with successful surgical removal has been presented.

A 19-year-old boy, 2 and half years before admis-sion, had severe head injury with left cerebral con-tusion which has been cured almost perfectly.

This time, he had a head blow fists and 19 days later, right subdural hematoma was found by cere-bral angiography. On operation, a tiny angiomatous malformation, with arachnoidal adhesion, was found under the subdural hematoma. And this malforma-tion was considered to bleed directly into the subdu-ral space.
