I.はじめに
末梢運動神経伝導速度の臨床的研究についてはすでに多くの報告がみられ,末梢神経機能検査法として重要である。しかしこの方法は筋肉の活動電流を介する間接的な測定であるので,筋肉の萎縮が高度で筋反応がえられない場合は測定不能である。かかる高度の筋萎縮を筋萎縮性側索硬化症や末梢神経損傷等にしばしば経験した。そこでわれわれは筋肉を介しない神経伝導速度を人の尺骨神経について記録しようと試み,臨床的および基礎的な二,三の事実について検討を試みた。
Conduction velocity of peripheral nerves has been studied utilizing evoked nerve pot-ential, stimulated at the distal portion and recorded at the proximal portion of the same nerve. The conduction velocity of nerve im-pulses might indicate those of larger fiver group in both afferent and efferent nerves. Therefore, tentatively it has been named as "Compound Nerve Conduction Velocity"( NC-V). Such NCV and the conventional motor ner-ve conduction velocity ( MCV) have been stu-diedon the ulnar nerves in normal individuals. motor neuron disease and peripheral neuro-pathies.
The NCV is smaller and less variable than those of MCV. Cases with motor neuron disease showed normal in NCV, while peri-pheral neuropathies showed diminished NCV.
It has been suggested that such method for NCV will be helpful in clinical evaluation of the peripheral nerve disorders.
