A 40-year-old housewife was clinically dia-gnosed as having hydrocephalus internus owi-ng to a subtentorial tumor, and a suboccipital trepanation was performed. The choroid ple-xus was replaced with a reddish-gray and papillomatous tumor in the 4th ventricle. The entire tumor, weighing 7g, was easily extir-pated.

Histologic study showed a typical feature of choroid plexus papilloma, eosinophilic, PAS positive, and large secretion granules were recognized in the tumor cells.

Even the inferior part of the rhombence-phalic groove, caudally from the tumor, was distinctly dilated, and the cisterna magna was exceedingly enlarged. On the basis of these two findings, together with the large secre-tion granules in the tumor cells, the present authors consider that the hypersecretory theory of choroid plexus papilloma is correct. However the marked hydrocephalus internus was probably caused mainly by the distur-bance of C. S. F. flow in this case.
