椎骨動脈に依る舌下神経根オリーブ核絞扼の像について

A PICTURE OF THE STRANGULATION OF RADIX NERVI HYPOGLOSSI AND NUCLEUS OLIVAE BY VERTEBRAL ARTERY

　椎骨動脈像に於いて腫瘍の局在と直接関係のない延髄部に変化を認めた。この変化は舌下神経根を後側方に圧迫しながら，オリーブ核を絞扼するような形で，椎骨動脈が延髄の側方に偏位している所の，脳圧及び腫瘍よりの圧追による二次的の変化である事を確めた。而してかかる場合には多かれ少かれ舌下神経障害が認められたが，これは椎骨動脈により舌下神経根が絞扼圧迫された為であり，腫瘍自身により舌下神経が庄迫されたのではなく，所謂Pseudofocal signであり，又かかる変化はTentorial herniation或いはその前段階に於ける脳幹の下方偏位を裏書きする一つの変化と見なし，臨床上注目すべきものであると考える。

In vertebral angiograms, authors could find a change which have no relation to the tumor localization. The change means late-ral displacement of vertebral artery pressing radix nervi hypoglossi and strangulating nu-cleus olivae in medulla oblongata, associated with the impairment of nervus hypoglossi as a clinical manifestation. Authors demon-strated such involvement in a case of fron-tal lobe tumor and in two cases of acoustic neurinoma (the change was observed in con-tralateral to the localization of the neurino-mas), and they emphasized that the change means downward displacement of brain stem and does not indicate the localization of ex-panding lesion in medulla oblongata.
