緒言
脳の代謝に於いて,炭水化物の代謝は特に重要な位置を占めるもので,従来種々の報告がある。例えば,低血糖に際し,脳機能が容易に障害され得る事は既に衆知の事実であるし,脳を灌流する動脈血・静脈血の両者中,最大の差を示すものは炭水化物代謝に関係する物質であり1),特にぶどう糖・乳酸の重要性が強調されている2)事も特に説明を要さぬ所であり,又我々が前級3)に於いて,脳の代謝中,ATPの重要性を述べた理由も,ATPが炭水化物代謝に密に関係しているからに他ならない。
我々は脳外傷に依り発生する諸種病態生理解明の一端に,神経化学面からの検討を意図しているものであるが,本論文では脳代謝の主要部分を占める炭水化物代謝を中心とした実験的脳外傷並びに臨床例の成績を報告する。
Pyruvic acid, α-ketoglutaric acid, ATP and inorganic phosphate were measured in exper-imentally injured brain in rats.
1) Pyruvic acid showed rapid increase imme- diately after the trauma with its maximum value in ten minutes both in mild and se- vere cases, and ATP showed generally rapid decrease with some exceptions after the trauma. The authors presume that these results are explanated by the normal anaer- obic decomposition of carbohydrate.
On the other hand decrease of α-ketoglu- taric acid and increase of inorganic phospha- te were observed in injured brain. These observations reveal that oxidation in TCA cycle is disturbed in traumatized brain.
II) The amount of pyruvic acid measured in both blood and cerebrospinal fluid of the patients with head trauma, showed a high- er increase in clinically severe cases than in clinically mild cases.
The amount of α-ketoglutaric acid meas- ured in cerebrospinal fluid showed none marked difference to the normal but the one in blood showed marked decrease.
