50year-old man complained of headache from the beginning of this disease. Successively several days later, he had onset of right spastic hemiparesis, diplopia, dysarthria, dysphagia, and retention of urine and feces. These symptoms were progressive. Examina-tion revealed paralysis of conjugate gaze to the upward and the left outward, ptosis of the right eyelid, loss of the right corneal reflex, upper motor neurone type of right facial palsy, horizontal nystagmus on gaze to the left and spontaneous nystagmus, loss of the right pharyngeal reflex, paresis of the right XI th and XII th nerve, moderate diminu-tion of deep and discriminative sensation, slight disturbance of right touch and thermal sense, hyperactive tendon reflexes and several positive pathological reflexes of the right extremities, disappearance of abdominal and cremasteric reflex, and cerebellar ataxia of the left extremities. Chocked disc, aphasia, apraxia and astereognosia were not observed. Cerebral angiogram, P. E. G. and E. E. G. were, however, normal.
