We report three cases with cluster headache treated with sumatriptan nasal spray. In all occasion, sumatriptan nasal spray showed meaningful relief in patients with cluster headache, although the efficacy were less than that of sumatriptan subcutaneous injection. Sumatriptan nasal spray needed 20 to 30 minutes to meaningful relief, while subcutaneous injection needed only 5 to 10 minutes. Recurrence of attack was more frequent in oxygen inhalation therapy than sumatriptan nasal spray. Sumatriptan nasal spray were constantly effective in both early stage and late stage of cluster periods.

 Sumatriptan nasal spray may shorten the admission period of severe case of cluster headache. Sumatriptan nasal spray may enable doctors to treat mild cases of cluster headache in their outpatient clinic.
