長期生存したアルツハイマー病の大脳皮質病変について

A Study of Cortical Lesions in Subjects who had Suffered from Alzheimer's Disease for a Long Period of Time

　抄録　長期経過をたどり失外套症状群を呈し剖検で高度の脳萎縮を示したアルツハイマー病の4症例を報告した。今回は神経細胞脱落を指標として大脳皮質病変を検討し，以下の結論を得た。神経細胞脱落はとめどなく進行し高度の脳萎縮を呈するに至るが，皮質連合野が著しく侵されるのに対して，一次性運動知覚野は最後までよく保たれ，従来からいわれている特徴が，さらに際立って表現されていた。文献の検討から，アルツハイマー病の疾患過程においては，症例により初期病変の局在パターンや領域別の病変形成の速度に相違があり，これが臨床徴候に反映すると考えられるが，このような経過を経た後に臨床的にも病理学的にも，ここに示した長期生存例のように共通した像を呈するに至ると考えられる。

　The detailed distribution of cortical neuronal loss was studied in four cases of persons who expired after long clinical courses of presenile Alzheimer's disease. All cases were in an apallic state for at least the last 2 years. The total duration of the disease ranged from 14 years to 17 years. All cases had marked cerebral atrophy. The brain weights were less than 800 gram. Although the whole cortex was more or less involved in neuronal loss, all cases showed a characteristic distribution pattern of neuronal loss.
