 The detailed distribution of cortical neuronal loss was studied in four cases of persons who expired after long clinical courses of presenile Alzheimer's disease. All cases were in an apallic state for at least the last 2 years. The total duration of the disease ranged from 14 years to 17 years. All cases had marked cerebral atrophy. The brain weights were less than 800 gram. Although the whole cortex was more or less involved in neuronal loss, all cases showed a characteristic distribution pattern of neuronal loss.
