 We present an autopsied case of Pick's disease, which was atypical both clinically and neuropathologically. A 57-year-old woman showed sensory aphasia as an initial symptom, and fell into total aphasia within 6 years. After that, personality changes and dementia developed. Neuropathological symptoms including hearing disturbance, muscle rigidity, nuchal dystonia and abnormal posture suggesting PSP also appeared. The patients became apallic, and died of bronchopneumonia 10 years after the onset of the disease. The neuropathological diagnosis was temporofrontal type of Pick's disease.
