抄録 昭和大学附属烏山病院では2011年より成人を対象として発達障害の検査を目的とした入院プログラムを提供している。検査入院に参加した患者175名のうち,22%が自閉症スペクトラム障害(ASD),21%が注意欠如多動性障害(ADHD)の診断であり,半数以上が発達障害ではないと判断された。前医での診断名と比較したところ診断の一致度は不十分であり,ASDは統合失調症と,ADHDはASDとみなされやすい傾向が示された。心理検査からはPARSの幼少期得点およびIRI得点,特に共感的配慮の下位項目が発達障害を疑われている他の精神疾患との鑑別に有用である可能性が示された。
Karasuyama hospital has provided short-term hospitalization program for detailed examination to adults with suspected neurodevelopmental disorders since 2011. So far, a total of 175 individuals participated in the hospitalization program. 22% of them were diagnosed as autism spectrum disorder(ASD) and 21% of them were diagnosed as attention deficit hyperactivity disorder(ADHD). Less than half of all participants were diagnosed as neurodevelopmental disorders. The diagnoses by the hospitalization program were often inconsistent with diagnoses by psychiatrist in primary care. Adults with ASD and ADHD were tended to be diagnosed as schizophrenia and ASD in the primary care, respectively. The scores of early childhood peak score in PARS(Pervasive Developmental Disorders Autism Society Japan Rating Scale) and IRI(Interpersonal Reactivity Index) were useful to find actual ASD among individuals who were suspected as ASD by primary doctors and themselves.
