 A case of ictal stupor in a 24-year-old man as characterized by frequent attacks of a stuporous state, accompanied by myoclonus, of 12 to 24 hours' duration and an EEG correlate of continuous generalized spike and wave complexes was reported.

 This case was of particular interest in the following five respects: 1) the attack, most characteristically, was accompanied by symmetric myoclonus involving muscles of the anterior chest and upper arms; 2) there was a close temporal relationship between the attack and the sleep-waking-cycle; 3) emotional affective responses acted as a precipitating cause of the attack; 4) the attack was suppressed by the intravenous injection of amobarbital and the benzodiazepine derivatives; and 5) the combined use of DPA and cloxazolam proved to be effective in the prevention of the attack.

 After differentiating the condition in question from epilepsia partialis continua and bilateral massive myoclonic status on the basis of the accompaniment of myoclonus, an attempt was made to place properly the present case in ictal stupor. From consideration of the five features enumerated above it was concluded that the limbic system and the reticular formation in the brain stem are concerned in the genesis of the condition. A study was also made of the pathophysiology of this particular disease state.
