精神科臨床現場における被害者・被災者支援の現状と課題―大学病院の過去10年間のPTSD患者の調査結果から

The Present Situation in Psychiatric Clinics and the Work to be Carried out in Support of Victims of Injury and Suffering

はじめに

　2004年12月に犯罪被害者等基本法が制定され，犯罪被害者などの心理的外傷，その他犯罪などにより心身に受けた影響から回復に向けての施策を講じる責務が国および地方公共団体に課せられた。医療機関においても被害者などへの医療をどのように担っていくかについて検討していくべき時期にさしかかっているといえる。

　実際，外傷後ストレス障害(PTSD)が医学的診断として定着して以来，主に精神科の臨床現場では，現在まで少なからぬ犯罪被害者や事故被災者例が精神科治療を受けるに至っている。しかしながら，臨床現場から研究報告として記述されたものは少なく，しかも多くは症例報告にとどまっているのが現状である。したがって本研究では，一大学病院における最近10年間のPTSD患者の受療状況を調査し，その病状・治療などの現状について明らかにするとともに，被害者あるいは被災者支援における精神科医療の課題について検討する。また，被害者の置かれている福祉的・法的問題にもふれ，それらのケアにかかわる医療従事者の問題について言及する。

　In recent years, patients with PTSD who are seeking medical treatment have increased in medical settings. To identify the current status of victim assistance, we investigated hospital records of all PTSD patients who visited Kurume University Hospital during the past 10 years. The most common complaint among the patients was sleep difficulty and about half of them were out of work or not at school. Furthermore, they often had serious economical problems after experiencing traumatic events such as sexual crimes. According to our results, we need to establish intensive collaboration among various facilities in order to provide them with medical treatment.
