DSAを用いた慢性肺気腫患者における肺血管床および血流調節異常評価の試み

Digital Subtraction Angiography:A technique for quantifying pulmonary vascular bed and flow regulation abnormalities

　肺細動静脈（直径100μm以下）や毛細血管により構成される肺血管床およびその予備能を評価する方法として，DSAを用いた肺動脈wedge angiographyの有用性を報告する．正常群5例，慢性肺気腫群5例を対象とした．通常の右心カテーテル検査後に，DSAによる肺動脈wedge angiographyを施行した．正常群では認められない肺血管床の減少が慢性肺気腫群では認められた．肺血管床に関心領域を設定し，time-density curveを作成したところ，局所の肺血管床の減少が定量的に評価可能であった．また，局所肺動脈に硝酸イソソルビドを注入することにより，肺血管床予備能も評価可能であった．本法を用いることによって，肺血管床の減少や予備能が評価可能であり，さらに低酸素性肺血管攣縮も視覚的・定量的に評価できる可能性が示唆された．

We report the usefullness of digital subtraction pul-monary wedge angiography to evaluate the pulmonary vascular bed and flow regulation abnormalities. 5 con-trols and 5 patients with chronic pulmonary emphysema were evaluated using the digital subtraction angiogra-phic technique. After right heart catheterization, digital subtraction angiography (DSA) was performed. Subtle defects in the capillary area, not seen in the controls, were defined in the patients with chronic pulmonary emphysema. Injection of isosorbide dinitrate enhanced this change more in controls than in the patients. By this technique, it is possible to evaluate quantitatively the flow reserve of the pulmonary vascular bed and the degree of hypoxic pulmonary vasoconstriction.
