　経皮的経管的肺動脈弁交連切開術はこれまで主として小児例に対して試みられており，成人例の報告は少ない．今回，われわれは肺動脈弁狭窄症に対し11年前に直視下交連切開術を受け，その再発で右室収縮期圧120mmHgと著明な右室負荷を来した1成人例に対し本法を施行した．本例は手術時の輸血によりC型肝炎を発症し，さらに右室負荷も加わり腹水貯留を伴う非代償性肝硬変症の状態で再手術は危険性が高いと考えられた．右室—肺動脈圧較差は術前90mmHgより50mmHgに低下させることができ，また臨床症状もNYHA3度より2度に改善した．本法は，簡便，安価，低侵襲，繰り返し施行可能で，治療効果も大きく，このような症例にも安全であり，優れた方法と思われた．

A 63-year old man with pulmonary valve restenosis underwent percutaneous transluminal pulmonary commissurotomy (PTPC). We regarded PTPC as a safe and effective procedure even for adults though there were few examples of this in the literature.

The patient had undergone open pulmonary commis-surotomy 11 years previously with residual type C hepatitis due to blood transfusion. Together with right ventricular hypertension (120 mmHg at systole) liver cirrhosis with ascites developed. This condition would make re-operation much riskier.
