冠攣縮を呈したcoronary ectasiaの1例

A Case of Coronary Ectasia in which Coronary Spasms were Induced

　冠攣縮を誘発しえたcoronary ectasiaの1例を経験したので報告する．67歳，男性，安静時胸部圧迫感あり冠動脈造影を施行した．右冠動脈本幹全体に著明な冠動脈拡張を認め，Ergonovine負荷により右冠動脈末梢分枝にspasmが誘発された．また，5ヵ月後労作時胸痛の出現をみ，再度冠動脈造影を施行したところ左冠動脈第1対角枝（＃9）に新たな器質的狭窄病変の出現を認めた．coronary ectasiaの症例では冠攣縮や急速な動脈硬化の進展をみることがあり注意が必要と考えられた．

Coronary spasms were induced by ergonovine loading in a case of coronary ectasia. A 67-year-old male underwent coronary angiography because of a feeling of compression in the anterior thoracic region. Marked coronary ectasia was noted in the entire trunk of the right coronary artery, and spasms were induced in peripheral branches of the right coronary artery by ergonovine loading. Five months later, repeat coronary angiography showed rapid progression of arteriosclero-sis in the diagonal branch of the left coronary artery (# 9).
