A 63-year-old woman, suffering from acute massive pulmonary embolism with congenital antithrombin Ⅲ (AT Ⅲ) deficiency. was treated successfully with mechanical clot fragmentation by using a catheter. The patient was admitted to our hospital because of dyspnea. Acute pulmonary embolism was suspected by arterial blood gas analysis, ECG, chest X-P, and echocardiogram. We performed right-sided cardiac hemodynamic assessment and pulmonary angiography immediately. They revealed marked pulmonary hyper-tension (94 mmHg in systole) and occlusion of the right upper and left lower lobe pulmonary artery segment.
