　先天性アンチトロンビンⅢ（AT　Ⅲ欠乏症を併存する肺塞栓症に対し，カテーテルによる血栓の破砕および血栓溶解療法により著明な改善が得られ，良好な経過を示した症例を経験した．症例は63歳女性．呼吸困難を主訴に来院．血液ガス分析，心電図，胸部X線写真，心エコー図検査より肺塞栓を疑い肺動脈造影を施行した．肺動脈収縮期圧は94mmHgと著明な肺高血圧を示し，肺動脈造影では左下葉枝，右上葉枝の閉塞，右下葉枝の欠損像を認めた．カテーテルによる血栓の破砕により左下葉枝，右下葉枝の開通が得られ，その後ウロキナーゼ72万単位を投与した．凝固・線溶系検査にてAT　Ⅲ活性が40％と低下しており，脳梗塞の既往を有する長女のAT　Ⅲ活性も55％と低下していたため先天性AT　Ⅲ欠乏症の一家系と判定した．肺塞栓症の治療として，従来の血栓溶解療法に加えカテーテルによる血栓の破砕が有用であることが示唆された．

A 63-year-old woman, suffering from acute massive pulmonary embolism with congenital antithrombin Ⅲ (AT Ⅲ) deficiency. was treated successfully with mechanical clot fragmentation by using a catheter. The patient was admitted to our hospital because of dyspnea. Acute pulmonary embolism was suspected by arterial blood gas analysis, ECG, chest X-P, and echocardiogram. We performed right-sided cardiac hemodynamic assessment and pulmonary angiography immediately. They revealed marked pulmonary hyper-tension (94 mmHg in systole) and occlusion of the right upper and left lower lobe pulmonary artery segment.
