慢性期正常冠動脈像を呈する梗塞後左心室瘤形成例の臨床的検討

Left Ventricular Aneurysm with Normal Coronary Arteriogram after Myocardial Infarction

　心筋梗塞後左心室瘤は高度冠動脈病変を伴うことが普通で，慢性期造影にて正常冠動脈像を呈する例は少なく，本邦でまとまった報告はない．我々は冠動脈造影の行われた症例についてその頻度，臨床像を検討した．冠動脈造影の行われた自験例連続1,800例中5例（0.28％）に正常冠動脈造影像を呈する梗塞後左心室瘤を認めた．年齢は34〜83歳（平均59歳），男性4例，女性1例であった．興味深いことに，どの症例も梗塞前狭心症の既往がなく最初の突然の胸痛で発症していた．心筋梗塞に至った機序として冠動脈血栓，スパズムの関与が疑われ，冠動脈スパズム誘発試験を行った症例で陽性所見を得た．また，左心室瘤形成には側副血行路の発達不良，残存心筋の良好な壁収縮力が関与している可能性もある．

Left ventricular aneurysms after myocardial infarc-tion are generally considered one of the complications of severe coronary artery disease. Postinfarction ventricular aneurysms with normal coronary arterio-gram are rare. Only a few cases have been reported previously in Japan. We examined the incidence and the clinical characteristics of postinfarction ventricular aneurysms without coronary obstructtion.
