LDL-apheresisにより冠動脈硬化の退縮したヘテロ型家族性高コレステロール血症の1例

A Case with Heterozygous Familial Hypercholesterolemia Showing the Regression of Coronary Atherosclerosis by LDL-apheresis

　67歳，女性のヘテロ型家族性高コレステロール血症（FH）患者にLDL-apheresisを施行し，2年6カ月後の冠動脈造影上退縮を認めた1例を報告した．

　患者は50歳頃より，眼瞼部，肘部の黄色腫に気づき，61歳時の胆石の手術時に初めて高コレステロール血症（＞350mg／dl）を指摘された．63歳時より降脂血薬の内服療法を受けていたが，薬物療法ではコントロール不良のため当科に紹介された．当科紹介時のchol値は398mg／dl（1カ月内服療法中断後）で，両足アキレス腱は肥厚（左18mm，右19mm）していた．次女と次女の長女に高コレステロール血症が見られ（それぞれ291mg／dl,312mg／dl），冠動脈造影上，Seg．1のprox-imalに90％，distalに96.8％，Seg．6に75％，Seg．13に81.5％の狭窄が見られた．当科でprobucol,cholestylamine,pravastatinの使用とLDL-apheresisを開始，2年6カ月後には，両足アキレス腱肥厚の（左右共14mm）退縮を示し，冠動脈造影上，Seg．1のdista1（96.8→74.6％），Seg13の（81.5→61.7％）退縮を認めた．

A case showing the regression of coronary atheros-clerosis by the treatment with LDL-apheresis, was report-ed. The patient was a 67-year old female with heter-ozygous familial hypercholesterolemia. She had noticed the xanthelasma or left cubital xanthoma at the age of 50 years old. She was informed about her high serum cholesterol level (>350mg/dl), and the abnormal thickness of her bilateral achilles tendon at the age of 61 years.
