　急性心筋梗塞（AMI）後の左室機能，左室remodeling過程に及ぼす再灌流療法の効果を検討した．前壁AMI75例を自然再灌流群（Rs群：12例），早期再灌流群（Re群：18例），後期再灌流群（Rl群：16例），非再灌流群（N群：29例）の4群に分け，急性期，慢性期左室造影より求めた左室容積，左室全体機能，梗塞部局所機能を比較した．慢性期左室拡張終期，収縮終期容積係数（LVEDVI, LVESVI）はRs,Re,Rl群でN群より有意に小であり，Re,Rl群間では差がなかった．慢性期左室駆出率（EF），梗塞部局所壁運動（RWM）はRs,Re群でN群より有意に良好であったが，Rl,N群間では差がなかった．急性期，慢性期左室造影を比較できた19例の検討では，N群（n＝10）でLVEDVI,LVESVIともに有意に増大したのに対し，Rl群（n＝5）ではLVEDVIは増大したがLVESVIは不変であった．RWMはRe群（n＝4）で有意に改善したが，Rl,N群では不変であった．以上より左室機能の改善が得られない後期再灌流でも左室remodeling抑制効果が示された．

To determine the relation between reperfusion ther-apy and left ventricular function and remodeling after acute myocardial infarction (AMI), 75 consecutive patients with anterior AMI were studied. The patients were divided into four groups according to the reper-fusion outcome and time to reperfusion from onset of MI: 12 patients with spontaneous reperfusion, 18 patients with early (＜4th) successful reperfusion, 16 patients with late (≧4th) successful reperfusion and 29 patients with unsuccessful reperfusion.
