大量喀血で発症した傍気管支血管腫性気管支動脈肺動脈瘻の1例

A case of peribronchial angiomatous malformation associated with massive hemoptysis

　傍気管支血管腫性気管支動脈肺動脈瘻（multiple peri—bronchial angiomatous malformation以下気管支動脈肺動脈凄と略す）は，比較的まれな疾患であり無症状に経過する例が多い。

　今回我々は，感冒後の頑固な咳漱に引き続いて大量の喀血を呈し，その原因として気管支動脈肺動脈瘻を認めた1例を経験した。症例は，当時24歳であり理学的所見・胸部X線写真・気管支鏡検査等では，喀血の原因となるような異常所見を認めず気管支動脈造影で初めて本疾患が確認された。このような体循環・肺循環吻合は，進行性であるといわれ，早期に手術もしくは経血管塞栓術が施行されている。本症例では，これらの処置に同意が得られなかったことから，その後経過観察下にあるが約6年以上にわたって無症状で推移している点，注目すべきものと思われる。

Twenty four year-old Japanese woman was ad-mitted to our hospital because of massive hemop-tysis induced by severe cough. Roentogenographic and bronchoscopic examinations did not show any significant findings, while bronchial angiography revealed anastomosis between the bronchial and pulmonary arteries with marked angiomatous net-work. It was thought that bleeding had occurred from this systemic arterio-pulmonary arterial mal-formation.

She has been followed up without any special treatments because she refused operation or trans-arterial embolization. However, she has had no further bleeding and been doing well for 6 years long.
