Recently, some vasodilators have been used in pul-monary hypertension. This study was designed to deter-mine whether dibutyryl cyclic AMP (DB-cAMP) could be used as pulmonary vasodilator in pulmonary hyper-tension. In 5 anesthetized open chested dogs, 5 repea-ted intermittent hypoxia (95% N2 and 5% CO2 venti-lation) was challenged to the left lower lobe. The electromagnetically measured flow diversion, which is an index of hypoxic pulmonary vasoconstriction, became progressively greater through the first 3 hypoxic challe-nges and became steady on the latter challenges. After 3 hypoxic challenges, we tested the same hypoxic challenge during infusion of DB-cAMP (0.1mg/kg/min and 1mg/kg/min). Systemic arterial pressure decreased but heart rate and cardiac output increased dose-depe-ndently by infusion of DB-cAMP. Hypoxic pulmonary vasoconstriction and arterial oxygen pressure also decreased dose-dependently.

We suppose that we can use DB-cAMP as pulmonary vasodilator in pulmonary hypertension because it in-hibits the hypoxic pulmonary vasoconstriction.
