Seventy-eight lobes from patients with cancer were histologically studied in order to investigate influences of ventilation disturbance (bronchial stenosis or pleural adhesion) and perfusion disturb-ance (arterial stenosis) upon structural changes of small pulmonary arteries. Intima of the small pul-monary arteries were thickened in 79% of ventila-tion-disturbed, but 11% of ventilation-undisturbed lobes. About a half of these thickening was due to proliferation of myointimal cells. Morphometric studies in 14 lobes with lung cancer revealed the following percentages of intimal and medial thick-ness ; 46% and 20% in both ventilation and perfu-sion disturbed lobes, 40% and 20% in only ventila-tion disturbed lobes, and 20% and 13% in lobes without disturbances. Those in emphysematous lungs were 41% and 15%, and 12% and 11% in seven control lungs. These findings suggested that focal hypoxic state induced by ventilation disturb-ance was one of the important factors of intimal and medial thickening of small pulmonary arteries in lobes with lung cancer, like those with emphy-sema.
