Mahaim束による間歇的早期興奮症候群の1例

A case of intermittent preexcitation syndrome using a Mahaim fiber

　心室早期興奮を示す副伝導路にはKent束，James束およびMahaim束があり，早期興奮症候群には顕性のものの他，間歇性および潜在性のものの存在が知られている。副伝導路の多く，とくに間性あるいは潜在性のものの大多数はKent束によるものであり，Mahaim束によると思われる間歇性早期興奮症候群の報告はまれである^1,2）。今回，我々は間歇的に出現するPQ短縮を伴わない早期興奮症候群の1例を経験したので若干の考察を加えて報告する。

A 18 year old man with intermittent ventricular preexcitation syndrome using a Mahaim fiber was rep-orted. The electrocardiogram showed regular sinus rhythm, PQ interval of 0.18 sec and nornal ST-T segment. The QRS wave showed intermittently small Δwave (QRS width 0.11 sec). On His electrocar-diogram, PA and AH intervals were 10 msec and 95 msec, respectively. ITV intervals were 25 msec in QRS wave with small J wave and 55 msec in normal QRS wave. From these findings, it was considered that small Δwave was produced by a Mahaim fiber.
