Mahaim束による間欠的早期興奮症候群の1例

A case of intermittent ventricular pre-excitation syndrome with a Mahaim fiber

　心室早期興奮が間欠的に出現する頻度は少なくないと報告されているが，この間欠的な出現は，Kent束，James束を副伝導路に有する場合がほとんどであり，Mahaim束が関与しているとの報告は極めて少ない。我々は，心電図で心室早期興奮を間欠的に認めた症例に電気生理学的検査を施行し，Mahaim束が関与しているとの所見を得たので文献的考察を加えて報告する。

A 61 year-old woman with intermittent ventricular pre-excitation syndrome using a Mahaim fiber was reported. The electrocardiogram showed QRS-com-plex with small δwave (100 msec in QRS width) or without small δwave (80 msec in QRS width). On electrophysiological study, during sinus rhythm, A-H interval was 100 msec. H-V intervals were 25 msec in QRS-complex with small δwave and 45 msec in QRS-complex without small δwave. From these fin-dings, it was considered that the small δwave was produced by a Mahaim fiber.
