第I度房室ブロック例における運動負荷心電図の有用性

　房室ブロックは，臨床上虚血性心疾患や，先天性心疾患，リウマチ性やジフテリア性心筋炎など器質的心疾患を有する人や，抗不整脈剤使用中または心臓外科手術後の人などの他に，健康人にも見られることがある^1〜7）。それゆえ房室ブロックの存在だけから，ただちに病的とはいえず，病的かどうかの判定には，体位変換や^8〜9），運動および薬物負荷試験が利用されている。

　今回われわれは心電図上第I度房室ブロックを指摘された症例にマスターダブル負荷試験を施行し，房室伝導機能について検討した。少数例ではあるが，ホルター心電図にて検討したところ若干の知見を得たのでその結果も併せ報告する。

First degree atrioventricular block (I°AV block) is known to occur in patients with coro-nary artery disease, congenital heart disease, rheumatic or diphteric myocarditis, during drug therapy, and after surgical manuplation to the A-V conduction system. However I°AV-block is also seen in normal individuals with vagotonia. Then it is important to distinguish whether I° AV block is due to organic atrioventricular conduction disturbance with organic heart dis-ease or increase of vagal tension.
