Morphometryからみた大動脈弁閉鎖不全時の左室肥大心の病態について

Histometrical estimation of the left ventricular hypertrophy in aortic insufficiency

研究目的

　開心術および大動脈弁置換術に心内膜下出血，心筋梗塞，"stone heart syndrome"など特異な心筋障害をおこすことがある。その原因として種々のことが推察されているが左室肥大心筋の虚血性傷害が主因とされ，大動脈遮断，術中間歇的冠灌流，体外循環中の心室細動，人工心肺の無血充填，微細血栓の冠動脈への塞栓などがこの合併症と関係すると考えられている。とくに大動脈弁閉鎖不全例の肥大心筋は容易に虚血性変化を受けやすいので，心筋の特異性を検索するためmorphometricに大動脈弁閉鎖不全の左室肥大の病態について検討した。

Fibrous tissues of endocardiac and epicardiac myocardium of the left ventricle was examined by means of Point Counting Method in an auto-psied heart with aortic insufficiency. Moreover, pathological changes in the tissues was examined with an optical microscope.

1. An increase in fibrous tissues in the endo-cardiac side was seen.
