血管造影上，冠動脈炎および心筋炎と思われる所見を有したneuro-vasculo-Behçet病の1例

A Case Report of Neuro-Vasculo-Behçet's Disease associated with Coronary Arteritis and Carditis in Cardiac Angiography

Behçet病は，口腔粘膜，皮膚，外陰，眼の4主症状に加え全身臓器の慢性反復性炎症を呈しつつ進行する疾患である。このうち，血管系に病変を有し，症状を発現するものをvasculo-Behçet病と呼び，しばしば重篤な合併症を呈したり，致死的になるため臨床上重視されている。さらに，Behçet病のうち心病変を有する例は，きわめてまれとされており，中でも冠動脈炎を呈した報告例は皆無である。われわれは，心筋梗塞様心電図を呈し，冠動脈造影検査の結果，冠動脈炎および心筋炎と診断されたneuro-vasculo-Behçet病の1例を経験したので報告する。

A 29-year-old man was admitted to our hospital with complaints of the swelling, redness and pain in his left lower extrimity. He was already diagnosed of complete type Behçet's disease. His electrocardiogram and vectorcardiogram showed the typical findings of inferior myocardial infarc-tion. The coronary arteriogram showed the ulcerating plaque in the distal and proximal portions of the right coronary artery. But, no significant stenosis was recognized in all coronary tree. The finding of inferior hypokinesia was seen in his left ventriculography. From these findings, we diagnosed that the coronary arteritis and carditis were existed as a part of Behçet's disease.
